Preparing for Appts The following guidelines will help ensure your appointment goes smoothly. Please check with your primary care doctors office for any specific instructions or preparations required for your appointment. You must confirm with our office if we received all referrals and pre-authorization prior to your first office visit. Before Your Appointment After we receive a referral or a request for an appointment we will call you to do a ''Pre-Visit Interview'', please have the following information available: Name Address Date of birth Social security number Insurance information Emergency contact information Confirm your health insurance coverage. Contact your health insurance provider to verify the specifics of your coverage. Make a list of the medications you currently take. Be prepared to discuss any prescribing information, such as how much you take and how often you take it. You will also provide us the Pharmacy you prefer to use. At the Appointment 1. Be prepared to pay any insurance copayments and/or deductibles 2. Bring the following information necessary paperwork: 3. All the forms: New Patient Form; Patient Info Sheet; Financial Policy and HIPAA Signed Policy 4. Your social security number 5. Insurance card, including your policy number and/or Medicare or Medicaid card 6. Valid ID as proof of identity (Drivers License or State Issued ID only) 7. Your employer's name, address and phone number (if you are insured through your employer) 8. Managed care referral authorization - this is necessary if you are covered by a managed care plan in which a physician referral is required 9. Name, address and phone number of your referring or primary care physician 10.Any forms you were asked to complete prior to the appointment 11.Any medical information related to your visit such as radiology results and lab diagnostic test results. This is especially important if you are coming for a second opinion. 12.Your complete medical and surgical history 13.A list of all medications, including prescription and over-the-counter medications, including dosage information 14.A list of any allergies to medicines, food, latex, dust, pollen or other sources 15.The names and telephone numbers of your primary care physician and any specialists you are seeing 16.A copy of your Health Care Proxy, if applicable During Your Visit you will: Complete forms about your health Be weighed and have your height measured Complete any tests requested by your doctor. These vary depending on your health and type of surgery, but can include vital signs, blood tests, urine tests and/or an EKG Complete a pre- and postoperative information about your surgery. Review your medical and surgical history Perform a physical examination Discuss your anesthetic plan You might also be seen by other medical specialists who will work together to prepare you for a safe surgical procedure.
